No Phone or Email?
You can still receive care. Walk in during clinic hours, or have a family member, case worker, shelter staff member, or hospital team complete the form for you.
You do not need insurance to get help.
If you cannot complete the online form, you can still come to the clinic in person during walk-in hours. The online form is optional and is meant to make scheduling easier — not to create a barrier to care.
